Doctors from Tulsa Bone & Joint and Ascension St. John Broken Arrow will be providing athletic physicals to Broken Arrow student-athletes and band members in grades 7th -12th, heading into the 2023-2024 school year, on Monday, May 8th, 2023 from 5:30 p.m. to 7:00 p.m. located in Broken Arrow High School’s Tiger Field House. The Sports Medicine staff asks that you contact your head coach to find out what time you will meet as a team. After meeting as a team, in a location that is provided by your head coach, you will walk you over to Tiger Field house together to check in at the following times:
- 5:30 p.m. – Football
- 6:00 p.m. – Cheer, Boys and Girls Basketball
- 6:15 p.m. – Volleyball, Softball, Baseball
- 6:30 p.m. – Girls & Boys Track Field / Cross Country, Girls & Boys Tennis, Boys & Girls Golf, Tigettes
- 6:45 p.m. – Boys & Girls Soccer, Girls & Boys Wrestling, Girls & Boys Swimming
- 7:00 p.m. – All Band Members
Cost to receive a sports physical is $20 and can be purchased through the following www.GOFan.co link: https://gofan.co/app/events/926365?schoolId=OK13167. There is a 2$ convenience fee when purchasing through GoFan.
www.GoFan.co registration will remain open through the day of the physicals, May 8th, closing at 4:30 p.m. Cash, Venmo, and CashApp payment will be accepted at the door, but it is strongly encouraged to pre-register and pay through www.GoFan.co. No refunds will be issued.
Students are also required to print, complete and bring this two-page physical evaluation form with them to their physical. We ask that all student athletes arrive in school appropriate athletic wear.
An athletic physical dated after May 1st, 2023, is required for students participating in Broken Arrow Public Schools athletics during the 2023-2024 school year.
Tulsa Bone and Joint and Ascension St. John physicians are providing the courtesy screening physicals. Additional follow-up may be required with your primary care doctor or other physician. All money collected is donated back to the Broken Arrow High School Athletic Training Program.
Tiger Field House is located on the west side of the BAHS campus at 1901 E. Albany.
Parking for physicals will be in the parking lot south of Tiger Field House.
