Physical Timeslots 2026 – 2026 PhysicalFlyer
Doctors from Tulsa Bone & Joint and Saint Francis will be providing athletic physicals to Broken Arrow
student-athletes and band members in grades 6th -12th, heading into the 2026-2027 school year, onMonday, May 11th, 2026 from 5:30 p.m. to 7:00 p.m. located in Broken Arrow High School’s Event Center.
The Sports Medicine staff asks that you contact your head coach to find out what time you will meet as a team. After meeting as a team, in a location that is provided by your head coach, you will walk over to the Event Center (enter in south doors from the new south parking lot off of county line/193rd St) together to check in at the following times:
· 5:15 p.m.
– Football
· 5:30 p.m.
– Cheer, Boys and Girls Basketball
· 6:00 p.m.
– Volleyball, Softball, Baseball
· 6:15 p.m.
– Boys & Girls Soccer, Girls & Boys Wrestling, Girls & Boys Swimming
· 6:30 p.m.
Golf, Tigettes
– Girls & Boys Track Field / Cross Country, Girls & Boys Tennis, Boys & Girls
· 7:00 p.m.
– All Band Members
If trying out for a team, please arrive at that team’s designated time slot
Cost to receive a sports physical is $25 and can be purchased through the following link: https://gofan.co/event/3466995?schoolId=OK13167_1
www.GoFan.co registration will remain open through the day of the physicals, May 11th, will be remaining open until the end of physicals. You must show proof of purchase with a copy of your receipt to enter the doors. Cash & Card payment will be accepted at the door, but it is strongly encouraged to pre-register and pay through www.GoFan.co. No refunds will be issued.
Students are also required to print, complete and bring this physical evaluation form

An athletic physical dated on or after May 1st, 2026, is required for students participating in Broken Arrow Public Schools athletics during the 2026-2027 school year.
Tulsa Bone & Joint and St. Francis physicians are providing the courtesy screening physicals. Additional follow-up may be required with your primary care doctor or other physician. All proceeds collected will go to Broken Arrow Sports Medicine.
The Event Center is located south of the football field (Memorial Stadium) of the BAHS campus at 2200 N 23rd St.
Parking is in the south parking lot (south of the football field). The entrance to this lot is located off of county line/193rd St.
